Initial Consultation

Contact us to schedule a FREE, no-obligation consultation. We will discuss your needs, preferences, and goals to determine how we can best support you.

During this initial meeting, our goal is to gain a comprehensive understanding of your unique needs, preferences, and goals. This consultation serves as a foundation for designing a personalised care plan that best meets your requirements

Care Plan Development

Our Care Managers will work with you to create a personalised care plan that outlines your goals, service requirements, schedule and routine, as well as health and safety protocols.

Based on the insights gathered during the initial consultation, our team will develop a personalised care plan tailored specifically to your needs and preferences. The care plan serves as a roadmap for delivering comprehensive and compassionate care.

Service Agreement

Once the care plan is agreed upon, we will provide a service agreement detailing the terms and conditions, including costs, service delivery, and responsibilities.

Commencement of Service

Upon signing the service agreement, our dedicated team will promptly initiate the delivery of the agreed services.

Introduction to Care Team

You will be introduced to your dedicated care team who have been selected based on their qualifications, experience, and compatibility with your needs and preferences.

Initial Service Implementation

Our team will start implementing the care plan as outlined, ensuring that all services are provided with professionalism, empathy, and respect for your privacy and dignity.

Ongoing Support and Communication

We maintain open lines of communication to address any questions, concerns, or adjustments you may have regarding your care plan or services. Regular check-ins and feedback sessions are scheduled to ensure your satisfaction and well-being.

Continuous Care Monitoring

We continuously monitor the effectiveness of our services and regularly update your care plan to reflect any changes in your health status, preferences, or goals
